Job Description:
A company is looking for a Pharmacy Technician, Site of Care
Coordinator to provide care coordination and health care navigation
assistance to members on specialty infused medications.
Key Responsibilities
Interact with patients and medical staff to explain benefits and
facilitate care transitions
Coordinate care and assist members in understanding benefit changes
related to specialty medications
Collaborate with health care providers to address gaps in care and
ensure uninterrupted therapy transitions
Required Qualifications
Valid Pharmacy Technician License per state requirements
National Certification - PTCB or ExCPT
Minimum two years of experience as a pharmacy technician
Previous telephonic experience in healthcare, preferably in
infusion or specialty pharmacy
High School Diploma
