Job Description:
A company is looking for a Care Manager (RN) to develop and
facilitate complex care management activities for members with
physical needs.
Key Responsibilities
Develop ongoing care plans and collaborate with providers to
address member's unmet needs
Identify barriers to care and implement appropriate care management
interventions
Coordinate between members, families, and care providers to ensure
person-centered care
Required Qualifications
Degree from an Accredited School of Nursing or a Bachelor's degree
in Nursing
2 - 4 years of related experience
RN - Registered Nurse - State Licensure and/or Compact State
Licensure required
Must reside in the PST zone
